Answering my own questions today.  Jim Schulman reviews it over at CG.  Had no idea that the item had been around since 2005.  He states that he achieves sufficient bars but he had to mod the cylinder to make espresso:

PS.  Orphanespresso still doesn't have the Handpresso Dome without Pods in stock.  Patience.
